FEES

The fees in our office are based on the care, skill, time and judgment necessary to provide the service. The fee for your particular treatment will be discussed fully with you prior to treatment. We make every effort to inform before we perform.

Payment for treatment is due at the time services are rendered. If other arrangements are necessary, this should be discussed with our business assistant prior to the appointment.

INSURANCE

We will do all we can to help you derive maximum benefits from your insurance company. In order for us to do this you must bring your dental insurance with the address of your dental insurance company.

Specialty exams and consults are often not covered; therefore, we expect you to be responsible for this fee at this visit.

WE have found that most insurance plans pay 50-80% of a preset schedule of fees established by the insurance company. Benefits are determined by the type of plan your employer has chosen. Once a fee has been established for the services needed we will be happy to file a preauthorization prior to treatment. This simply tells the insurance carrier what we plan to do and what it will cost. They will then advise you and us in writing exactly what they will pay on the treatment. Only after we have this information will we acceot insurance assignment of benefits for payment or partial payment.

Keep in mind that most policies have a deductible and a yearly maximum.

PAYMENT OPTIONS

We accept Visa and Mastercard, ATM cards, cash, and personal checks.

If your insurance co-pay is more than you can handle with one payment , we can usually arrange an extended payment plan. If this is necessary please discuss with business assistant prior to treatment.
